So even if your friend or coworker had their procedure covered through Blue Cross, that doesn’t automatically mean your Blue Cross plan will do the same. Why? Because benefits can be customized by the employer, and not every employer opts to include weight loss surgery in their plan. And also because specific criteria for each individual must be met.

What Insurers Typically Require For Bariatric Surgery Coverage

Even if your plan does cover bariatric surgery, approval is not automatic. You have to meet strict criteria set by your insurer. These typically include:

1. BMI Thresholds

Most insurers require a Body Mass Index (BMI) of:

  • 40 or higher, or
  • 35 or higher with one or more obesity-related conditions (such as type 2 diabetes, sleep apnea, hypertension, or severe joint pain).

2. Documented Weight Loss Attempts

You’ll usually need to show that you’ve tried other weight loss methods before turning to surgery. This might include:

  • Medically supervised weight loss programs
  • Nutritionist visits
  • Lifestyle changes documented by a physician

Some insurance plans even require a 3–6 month supervised weight loss program before they’ll approve surgery. Idaho BMI has a medically supervised weight loss program available. Medicare and Medicaid For Bariatric Surgery: A Different Set of Rules

Medicare Coverage For Bariatric Surgery

If you’re a Medicare recipient, there’s good news: Medicare does cover bariatric surgery for qualifying patients. However, the surgery must be performed at a Medicare-approved facility, and patients still have to meet medical necessity criteria. Idaho BMI is a Medicare-approved facility.

Medicaid Coverage For Bariatric Surgery

In Idaho and many other states, Medicaid may cover bariatric surgery, but it depends on budget allocations and clinical guidelines. Approval can be harder to get and often requires extensive documentation and pre-authorization. Idaho BMI does accept medicaid, however some specifics may apply.

Unfortunately, denials are common—even for patients who seem to meet all the criteria. Sometimes it’s a paperwork issue, other times it’s due to coding, and in some cases, it’s simply that the plan doesn’t include bariatric benefits.
